Dispatch a clean item

Clean items leave the queue by email — one email per item, with the split PDF attached — after the grace window, honouring the configured dispatch mode (item_ready or document_complete).

Steps

1. Confirm the item is reviewed and matched. (TODO)
2. Understand the active dispatch mode and grace window (set by a Reviewer). (TODO)
3. Let dispatch run, or trigger it. (TODO)
4. Verify it went out in the Outbox / mailbox. (TODO)
5. If it didn't, see Dispatch didn't go out.
